Project Description

With support from the California Arts Council, PATH will expand and formalize our “Coming Home to Music” program, in collaboration with Street Symphony, to connect musicians and formerly homeless individuals with chronic physical and mental health issues through classical music performances and dialogue. The performances create a safe environment for musicians and residents to discuss the emotional impact of the music and form meaningful connections with one another.

Organization Summary

PATH empowers people experiencing or at risk of homelessness with the tools they need to move into permanent housing, rebuild their lives, and thrive. To address this population’s complex needs, PATH uses several core approaches: strategic outreach conducted by MSW- and BA-level staff with mental health and substance abuse expertise; individualized, housing-centered case management; a “whatever it takes” approach to help clients successfully navigate the myriad services that exist to help them achieve self-sufficiency; interim beds to provide stability while individuals access the services needed to secure permanent housing; placement into housing through our network of landlords, Veteran housing vouchers, and 1,150 units of permanent housing; and services to support the goal of permanent housing, including employment services, benefits assistance, and linkages to healthcare. Since 2013, PATH has helped more than 9,000 people secure permanent housing across California
